If you have an example of a needs assessment questionnaire that you used at your institution to justify a new Neuroscience degree program proposal, I would greatly appreciate receiving a copy from you.

I am on a committee proposing a Neuroscience minor, and we are at the point where we need to gauge the interest among current students in such a program.
