Hi Rory,

I have used something like the attached for assessing a specific module.
Q1-Q15 were given as a pre & posttest, which is a design that I would
recommend for measuring gains. I'd also get the students' demographics on
both the pre and posttests (items D1-D8 on this form). Finally, as a part
of the posttest, we had a variety of survey items (E1-E10 on this form).

I'm working on a more general test of neuroscience if you are interested.
